Lepr, short for leptin receptor, is a key component in the leptin signaling pathway. Leptin, a hormone protein, regulates food intake and energy expenditure, and Lepr is essential for leptin to exert its functions, thus playing a vital role in maintaining energy homeostasis [8].
A meta-analysis including 16 individual studies with 7827 subjects suggested that LEPR Gln223Arg polymorphism may not affect the susceptibility to type 2 diabetes (T2DM) [1]. Another meta-analysis showed that rs1137101 (p.R223Q) in LEPR was significantly associated with T2DM in all genetic models, while rs1137100 (p.R109K) and rs8179183 (p.K656N) were not significantly associated with T2DM [8].
In the context of cancer, a meta-analysis of 5819 cases and 8068 controls found that the LEPR K109R (rs1137100) genetic polymorphism did not significantly affect the overall cancer risk, but was associated with breast, lung, and gastric cancers in specific genetic models [4]. LEPR hypomethylation was significantly associated with gastric cancer in males, and it might also have the potential to predict recurrence in GC patients [2]. For colorectal cancer in the Mexican population, the c.326A>G heterozygous genotype of LEPR revealed an increased risk, while the AG haplotype had a protective effect [5].
In Chinese Mulao people, rs1137100, rs2767485, and rs465555 of LEPR were associated with osteoporosis [6]. In the Ukrainian population, the presence of the LEPR gene (rs1137101) polymorphism indicates an increased risk of the metabolic syndrome development [7]. Monoallelic pathogenic LEPR variants were functionally tested and do not elevate the risk of obesity or BMI levels [3].
In conclusion, Lepr is crucial for energy homeostasis regulation. Studies on Lepr gene polymorphisms and methylation have revealed its associations with various diseases such as T2DM, different types of cancers, osteoporosis, and metabolic syndrome. Functional studies of Lepr, especially those on its gene polymorphisms, contribute to understanding the genetic basis of these diseases and may provide potential targets for disease prevention and treatment.
