Ica1l, which has sequence similarities to ICA69 (also known as ICA1), is a gene with diverse functions. It forms BAR-domain complexes with PICK1, and this interaction is crucial for acrosome formation in spermiogenesis. Ica1l is expressed in testes and brain, and in testes, it is the major binding partner for PICK1 [3].
ICA1L has been associated with several diseases. In genetic studies, it was found that the expression of ICA1L is associated with lacunar stroke, a subtype of small-vessel stroke, through transcriptome-wide association study (TWAS) [1]. An integrative analysis also revealed that ICA1L was associated with small-vessel stroke (SVS) based on brain-derived data at both protein and transcriptional levels [2]. Additionally, a proteome-wide association study (PWAS) showed that ICA1L is significantly associated with SVS and non-lobar intracerebral haemorrhage (ICH), and the SMR results indicated a causal association of its protein expression levels in the brain with these traits [4]. In cellular myoid neoplasms, novel SRF-ICA1L fusions were identified in some spindle cell tumors, which may define a subset of neoplasms composed of immature smooth muscle cells and have potential for malignant behavior [5]. In the context of neurodegenerative and neurodevelopmental disorders, ICA1L was identified as a potential drug target in studies related to attention deficit hyperactivity disorder (ADHD) and Alzheimer's disease. In ADHD, it provided protection against both ASD and ADHD, while in Alzheimer's disease, its brain protein abundance was found to be causal, but it was not recommended as a prioritized drug due to potential side effects [6,7].
In conclusion, Ica1l is important for acrosome formation during spermiogenesis. In disease contexts, Ica1l has been linked to various cerebrovascular and neurological conditions, such as small-vessel stroke, intracerebral haemorrhage, ADHD, and Alzheimer's disease, highlighting its potential as a biomarker or drug target in these areas. The study of Ica1l using genetic models, such as the ICA1L-knockout mouse model which demonstrated its role in sperm development, helps in understanding its function in both normal biological processes and disease conditions [3].
